Heart of NM Case Management (Heart of NM) is a provider agency that works exclusively for individuals with developmental disabilities who have been allocated to the NM Developmental Disabilities Waiver & Supports Waiver.
Heart of NM promotes the development of a society that fully includes individuals with disabilities by monitoring and ensuring services are provided in a timely and effective manner in order to meet all physical, spiritual, and psychosocial needs by exercising their right to make choices, grow, and contribute to their community.
Heart of NM will help shape a community where employment opportunities are available to people with disabilities and collaborate with community partners to encourage the development and expression of our clients’ unique potentials.
